5. One (1) piece long white folder with fastener;

FOR TRANSFEREE:

1. Original Honorable Dismissal;
2. Authenticated copy of Transcript of Record (TOR);
3. Authenticated copy of Birth Certificate from NSO/PSA (to be returned), and 1 photocopy of it;
4. Two (2) copies of ID picture 2”x2” size with white background;
5. One (1) piece long white folder with fastener;

FOR RETURNEE:

1. Last Grade attended;
2. Certificate of Registration (COR);

FOR SECOND DEGREE:

1. Authenticated copy of Transcript of Record (TOR);
2. Authenticated copy of Birth Certificate from NSO/PSA (to be returned), and 1 photocopy of it;
3. One (1) piece long white folder with fastener;
4. Two (2) copies of ID picture 2”x2” size with white background;

FOR GRADUATE SCHOOL (Masteral Degree and Doctoral Degree):

1. Transcript of Record (TOR) - 2 copies;
2. ID picture 2”x2” size - 2 copies;
3. Grade Point Average (GPA) - 1.75;
4. Recommendation Letter from the Previous Professor - at lease 2 professors (If Doctoral Degree);
5. Qualifying Exam - Payment of 300 non-refundable;
6. Interview;
7. Permit to Study - From the school/agency you are working;
8. Letter to the Dean;
